Prescription Refills

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's prescription by submitting the following form. Please be sure to fill in all the requested information, including a phone number where you can be reached. All approved prescription refills will be ready for pick-up the next business day (Monday-Friday) by 12 noon.

We also offer the option of having your pet's medication refill shipped directly to you. Please allow 7-10 business days for your prescription to arrive. Shipping and handling fees are $5. If you would like to take advantage of this option, please indicate so in the 'Additional Comments' section of the form below.
